Confirmation Registration

Thank you for choosing to receive the sacrament of Confirmation at St. Clare's Parish. Please download, print and complete the registration forms below. Forms can either be dropped off at the office during office hours or emailed to stclaresto.archtoronto.org.

REQUIREMENTS CHECKLIST

Please note that the office will not accept partial paperwork. All requirements listed below must be received at the same time for your child to be enrolled in the Confirmation program. Thank you.

• Registration Form
• $100 Fee (Cash or Cheque ONLY)
• Baptismal Certificate (Note: if your child is not baptized, please speak with Fr. Mariusz)
• Sponsors’ Confirmation Certificate
• Media Release Form

Registration Forms will not be accepted after October 3, 2025. No exceptions. 

REQUIREMENT FOR SPONSORS:

The Code of Canon Law establishes the following. Sponsors must:

• Be at least 16 years old, Baptized and Confirmed in the Catholic faith.
• Be an active Catholic: “An active Catholic is one who attends Sunday and Holy Day Masses regularly and participates in the Sacramental life of the Church.”
• Not be a parent of the candidate.
• The confirmation sponsor’s role is one of a mentor in the Catholic faith. He/she will present the candidate to the Bishop/Priest on the day of Confirmation, and in doing this, ensure that the candidate is fully prepared to not only receive the sacrament of Confirmation, but also live the Christian life. The sponsor should continue to support the candidate throughout their lives.
