First Communion Registration

Thank you for choosing to receive Holy Communion at St. Clare's Parish. Please download, print and complete the registration forms below. Forms can either be dropped off at the office during office hours or emailed to stclaresto.archtoronto.org.

REQUIREMENTS CHECKLIST

Please note that the office will not accept partial paperwork. All requirements listed below must be received at the same time for your child to be enrolled in the Confirmation program. Thank you.

• Registration Form
• $100 Fee (Cash or Cheque ONLY)
• Baptismal Certificate (Note: if your child is not baptized, please speak with Fr. Mariusz)
• Sponsors’ Confirmation Certificate
• Media Release Form

Registration Forms will not be accepted after October 3, 2025. No exceptions.
